CASE STUDY: Caregiver Role Strain: Ms. Sandra A.

Sandra, a 47-year-old divorced woman, received a diagnosis of stage 3 ovarian cancer 4 years ago, for which she had a total hysterectomy, bilateral salpingo- oophorectomy, omentectomy, lymphadenectomy, and tumor debulking followed by chemotherapy, consisting of cisplatin (Platinol), paclitaxel (Taxol), and doxorubicin (Adriamycin). She did well for 2 years and then moved back to her hometown near her family and underwent three more rounds of secondline chemotherapy. She accepted a less stressful job, bought a house, renewed old friendships, and became more involved with her two sisters and their families.

Sandra developed several complications, including metastasis to the lungs. Then she could no longer work, drive, or care for herself. She had been told by her oncologist that there was nothing else that could be done and that she should consider entering a hospice. She met her attorney and prepared an advance directive and completed her will. She decided to have hospice care at home and, with the help of her family, set up her first floor as a living and sleeping area. She was cared for by family members around the clock for approximately 3 days.

Sandra observed that she was tiring everyone out so much that they could not really enjoy each other’s company. At this time, she contacted the Visiting Nurse Association (VNA) to seek assistance. Her plan was to try to enjoy her family and friend’s visits. After assessment, the VNA nurse prioritized her problems to include fatigue and caregiver role strain. Other potential problem areas that may need to be incorporated into the care plan include anticipatory grieving and impaired comfort.

In this article, Mezirow (2003) makes a distinction between “instrumental” and “communicative” learning. “Instrumental learning” refers to those processes which measure and gauge learning, such as tests, grades, comments, quizzes, attendance records and the like. “Communicative learning,” on the other hand, refers to understanding created over time between individuals in what Mezirow calls “critical-dialectical-discourse,” (p. 59) which is a fancy way of saying, important conversation between 2 or more speakers. Another key idea Mezirow discusses is “transformative learning,” (p. 61) which changes the mind, the heart, the values and beliefs of people so that they may act better in the world. On the one hand, he is right: there are some people who cannot fully engage because their crisis is so long and deep, they are prevented. One thing is certain: if we gave as much attention to the non-instrumental forms of intelligence–like goodness, compassion, forgiveness, wonder, self-motivation, creativity, humor, love, and other non-measured forms of intelligence in our school curriculums, we’d see better people, actors in the world, and interested investigators than we currently have graduating high school.

Brief synopsis of the research article you selected.

The article I chose Harm Reduction and the American Difference: Drug Treatment and Problem-Solving Courts in Comparative Perspective by Noland. This article talked about drug courts and how others like them are called problem solving courts. These courts are designed not to punish people but to help them solve a problem. For example, a drug court would be focused on helping people get off drugs instead of throwing them in prison.

These courts operate on the harm reduction philosophy. They all have five common features first; they all have close ongoing judicial monitoring. Second, they all have a multidisciplinary or team approach. Third, they are all treatment oriented. Fourth, they all alter the traditional adjudication process. Finally, they all put an emphasis on solving the problems of offenders (Nolan, 2010).

The harm reduction model states treatment is a success if less harm is done because of going through treatment (Geyer, 2009). According to this treatment a person can get put back in jail or prison and still be considered a success. If what they got sent back for was not the same as before. For example, if Henry goes back to prison for possession of crack. His charge before was possession of marijuana. He would be considered a treatment success because he did not go back for possession of marijuana.

I like the harm reduction model because it has the most options for success when compared to the other two models. With this model a person has a much better chance of success than with any other model. I believe more options lead to more success. Another example of the harm reduction model would be in Australia they have three types of sex workers. They have the ones that are call girls, ones that work in brothels, and ones that work the street. In the courts there they try to take the ones from the streets and put them to work in the brothels. This is harm reduction because they are taking women off the streets and putting them in better working conditions. Yes, they are still prostituting but they are safer in a brothel than on the street (Nolan, 2010).
