1)INTRODUCTION

· Provide a brief overview of the public health significance of the issue your plan addresses, and why it is important to intervene within the population and setting you have chosen.

· Briefly describe what the plan will accomplish and how it is organized.

Epidemiological Assessment:

· Define your public health issue and describe the positive and/or negative health outcomes related to this issue. If your issue is a health outcome (e.g., Type 2 Diabetes), describe the other positive and negative health outcomes associated with it. If you are focusing on a health-related behavior (e.g. breastfeeding, vaping), describe the health outcomes associated with the behavior.

· Provide information regarding the distribution of your public health issue/behavior and related health outcomes by age, sex, ethnicity, location, and other variables you deem important. Use appropriate epidemiological data and statistics to provide a strong rationale for focusing on a specific population (e.g., versus other groups), geographic area, etc., or for focusing on a universal or non-specified population. If data does not exist for your exact population, geographic area, etc., use data from which you can extrapolate and explain both your rationale for using those data and the shortcomings of your extrapolation.

· Include at least one goal and one objective for your targeted health outcome(s). If appropriate, provide a rationale for the goal(s) and objective(s) selected. Make sure that the goals and objectives for each outcome are linked

· Identify indicators related to the targeted health outcome(s) that can be used in the baseline assessment and for evaluation. Identify the secondary data that you will be using and when you will access and collect it.

2) Social Assesment

· Describe the socio-demographic characteristics of your priority population and setting.

· Identify the social and economic and other quality of life issues, as well as values, assets, and capacity in your population.

· Explain how you will engage project stakeholders as active partners in the planning, assessment, implementation, and evaluation processes.

· You are not required to collect primary data for this project; however, you are welcome to include discussions with community representatives or other key informants.

· Describe what information is missing and include a plan to collect secondary data to address these identified gaps.

· Include at least one quality of life goal and associated objective related to your plan. If appropriate, provide a rationale for the goal(s) and objective(s) selected. Make sure that the goal(s) and objective(s) are linked.

· Identify indicators related to the identified quality of life outcome(s) that can be used in the baseline assessment and for evaluation. Describe how and when you will collect the secondary data.

· Identify and create an inventory of behavioral and environmental (physical and social) determinants (factors that are associated with, influence, or cause) your targeted health outcomes or behavior specified in Module 1.

· Assess the importance and changeability of each behavioral and environmental determinant. You may also include other prioritization criteria (e.g., increasing trends; disparities).

· Prioritize behavioral and environmental determinants that you have identified. Provide a clear rationale for why you prioritized as you did, using empirical data, theory and/or evidence from the literature, etc., as well as a description of your prioritization process. You may want to prioritize the behavioral and environmental determinants separately, together, or using an iterative process.

· Include at least one goal and one objective for each prioritized behavioral and environmental determinant. Make sure the goals and objectives for each of the determinants are linked.

· Identify indicators related to these determinant-related objectives that can be used in the baseline assessment and for evaluation. Identify the data sources for these indicators. Describe how and when you will access and collect the secondary data.

· Identify all possible precursors or antecedent factors that are linked to the prioritized behavioral and/or environmental determinants from Module 3. Be sure to explore precursors from all levels of the social ecological model, as well as both risk and protective factors. If you are using the PRECEDE model, specific whether the precursors are predisposing, enabling, or reinforcing factors.

· Include a conceptual model of your health issue, which clearly depicts the assumptions underlying the relationships among your behavioral and environmental determinants and their associated precursor and your targeted health issue. Be sure to give a brief description of the conceptual model in the text to clarify the causal pathways and describe how the model is grounded in social/behavioral science theory.

· Rate each precursor on importance/changeability and other criteria (if any) you have selected, using evidence from the empirical and grey literatures and (if applicable) theory.

· Set priorities within the categories or organizational framework (e.g. PRECEDE) for the precursor/factors that you listed. You may want to use a table to show your inventory, your ratings, related literature, and your priorities. At the end of the process, you should have a coherent set of interrelated precursors upon which to focus your intervention activities.

· Include at least one goal and one objective for each of your prioritized precursors. Make sure the goals and objectives are linked.

· Identify indicators related to these precursor-related objectives that can be used in the baseline assessment and for evaluation. Identify the data sources for these indicators. Describe how and when you will access and collect the secondary data.

· Identify existing evidence-based interventions (EBIs) and/or ‘promising programs’ or ‘best practices’ appropriate for your public health issue.

· Select the appropriate intervention strategies, activities or methods that are most likely to produce the desired changes in the target population that you described in your goals and objectives. You may use appropriate pre-existing evidence-based interventions, adapt one or more EBIs, or develop your own intervention, if no appropriate EBI exists or can be adapted for your public health issue. Be sure to consider the population for which the EBI has been determined successful in relation to the population you are focusing on. Be realistic about what a particular intervention can achieve. Cite evidence to support that your chosen intervention methods can achieve the desired changes.

· Include a conceptual model of your intervention (a.k.a. ‘program theory’), which clearly depicts the assumptions underlying your intervention. Be sure to give a brief description of the conceptual model in the text to clarify the casual pathways and describe how the intervention is grounded in social/behavioral science theory.

· Identify potential political, economic, historical, and/or structural barriers that must be taken into account.

· Identify the contextual issues that might have an impact on your intervention. These factors include organizational administrative structure and policies, as well as local or national laws, policies, and regulations (from founders, public agencies, and others).

· Specify potential collaborators for the elected intervention strategies, and describe why their inclusion will enhance the likelihood of the intervention’s success.

· Describe your intervention activities and how they will be implemented, including a timeline for the intervention activities and the administrative tasks necessary to implement the program plan.

· Include a logic model for the overall program, which includes elements for each of the intervention strategies.

· Describe how you will assess program implementation. Identify indicators related to your process objectives and identify the data sources for these indicators. Identify the data sources for these indicators. Describe how and when you will access and collect the secondary data.

· Describe how you will incorporate the results of the process evaluation into ongoing program activities and program revisions.

· Describe the budget required to plan, implement, and evaluate the intervention specified in the program plan. Include a brief narrative budget justification, as well as the overall (big picture) budget figures. Be as specific and as realistic as possible.

· In the budget narrative, describe the staffing necessary for the intervention, including their qualifications, responsibilities, and percent effort and duration required for each staff person, including volunteers, when appropriate.

· Provide a project timeline that depicts major intervention activities and milestones.

· Briefly describe how you will assess program implementation, effectiveness and impact, and who will be responsible for the assessment/evaluation activities. Note that you do not have to design a program evaluation, however you do need to show that you have ‘set up’ your intervention for evaluation by planning for collection of appropriate data at appropriate intervals before, during, and after the intervention activities.

· Include goals and measureable (SMART) process and outcome objectives for your intervention. These objectives should be the same or revisions or adaptations of the objectives you developed along the way for your health issues, behavioral and environmental determinants, and precursors.

· Describe how, when, and to whom you will disseminate the outcome evaluation results, and how the results will be used.

· Describe how you will insure program sustainability.
