OPERATING AGREEMENT CONTRACT BETWEEN
DORA STAFFING LLC AND CAMELIA ROSE CARE CENTER
I. LIABILITY on Operating Agreement Contract DORA
DORA STAFFING LLC is a Limited Liability Company formed under the laws of the State of
Minnesota. The undersigned hereby adopt the following Operating Agreement pursuant to the
LLC laws of the State of Minnesota, and do hereby certify and agree as follows:
It is understood that Dora Staffing LLC is to engage to provide proficient, trained and
professional RNs, LPNs, TMAs and CNAs according to the Minnesota employment
standard as needed to cover any available shift offer by Camelia Rose care center.
It is understood that Dora Staffing LLC is only liable for services performed by assigned
healthcare worker and cannot be held liable for the services performed after hours of
services.
It is understood that Camelia Rose Care Center will assign appropriate job task to license
personnel according to the employee title/license. Camelia Rose will also provide proper
supervision on site and will immediately report any misconduct according to the state
policy.
All employees will be responsible to provide service according to their license and will
be responsible to verify that jobs assigned matched their on hand license.
II. CALL IN POLICY
Dora Staffing LLC will follow up with any employee call in ASAP to allow facility to
book a replacement.
Representative agent will be responsible to set up and follow up with employee schedule.
Employee will cover full shift ( 8 hours with a 30min break) with no exception unless
otherwise specify by Facility staffing manager.
Facility staffing Manager will ensure that workers cover at least 4 hours shift before
leaving the facility once on site.
III. PAYEMENT SCHEDULED AND METHOD
Monthly payment is to be made to the company account (information will be provide on
due time) and a check summary to be mail at the company representative address 12640
Hanson Blvd NW Coon Rapids MN 55448 before the 5 th of each month.
Eligible benefit per employee will be perceived as defined by Minnesota Department of
health SNSA maximum allowed charges effective January 1 st 2020 (see attached
document).
13.3 Amendments and Revocations: This Operating Agreement may be amended or revoked at
any time by the written consent of all of the party.
13.4 State Law: This Operating Agreement will be governed by, construed, and enforced in
accordance with the laws of the State of Minnesota.
THE UNDERSIGNED, being all of the parti involved, evidence of their adoption and ratification
of the foregoing Operating Agreement of the LLC.___________
